Transition and re-integration for young people into the most appropriate setting to meet their needs is our ultimate goal and mark of success. Transitions at any time can be hard, but even harder after a significant period of time away for their formal setting. 

After building a successful working relationship during the 1-1 support phase, strategies and techniques identified can then be implemented and tested during this important phase. This most important part of a young persons educational journey can once again begin with the knowledge of knowing we are there to help an support where necessary.
